Securing A Birth Certificate

Helpful Tips in Securing a Birth Certificate

FOR UNMARRIED COUPLE WITHOUT FATHER’S ACKNOWLEDGMENT

STEP 1: Sign the Birth Certificate form Article 176 of the Family Code states:

“If the father does not recognize the child, the entry for middle name in the birth certificate must be left blank and the illegitimate child shall use the surname and shall be under the parental authority of the mother.”

STEP 2: Provide cedula The cedula must be of the recent year which will be presented to the Local Civil Registrar Office for registration.

STEP 3: Pay required Fee Payment is required before processing the birth certificate. You will be charged: ₱ 100.00

STEP 4: Wait for further instruction HIM (Medical Records) Personnel will instruct you about the process flow of the certificate.

STEP 5: Complete the Clearance Form Settle your accounts at the Cashier. The cashier will sign the Patient’s Clearance Form which will be required before the release of the birth certificate.

STEP 6: Receive Records The HIM (Medical Records) Personnel will contact you once the certificate is ready for pick-up. Releasing of the certificate depends on the processing time at the Local Civil Registrar Office.

*The mother must provide the needed requirements within 30 days from the birth of the baby. Additional ₱ 100.00, charged by the LCR Office, for late registration shall be shouldered by the parents.

FOR UNMARRIED COUPLES WITH FATHER’S ACKNOWLEDGMENT

STEP 1: Sign the Birth Certificate Ensure that all information supplied is true and correct.

STEP 2: Sign an Affidavit of Acknowledgment/Admission of Paternity Article 176 of the Family Code states:

“Illegitimate children shall use the surname and shall be under the parental authority of their mother, and shall be entitled to support in conformity with this Code. However, illegitimate children may use the surname of their father if their filiation has been expressly recognized by the father through the record of birth appearing in the civil register, or when an admission in a public document or private handwritten instrument is made by the father.”

 STEP 3: Provide cedula (both parents) The cedula must be of the recent year which will be presented to the Local Civil Registrar Office for registration.  

STEP 4: Pay required Fee Payment is required before processing the birth certificate. You will be charged: ₱ 100.00

STEP 5: Wait for further instruction HIM (Medical Records) Personnel will instruct you about the process flow of the certificate.  

STEP 6: Complete the Clearance Form Settle your accounts at the Cashier. The cashier will sign the Patient’s Clearance Form which should be presented to HIM (Medical Records) personnel.

STEP 7: Receive Records The HIM (Medical Records) Personnel will contact you once the certificate is ready for pick-up. Releasing of the certificate depends on the processing time at the Local Civil Registrar Office.

*The parents must provide the needed requirements within 30 days from the birth of the baby. Otherwise, the parents will pay additional ₱ 100.00 for late registration to the LCR.

FOR LATE REGISTRATION OF THE NEWBORN

The requirements are:

  1. Four (4) copies of the Certificate of Live Birth duly accomplished and signed by the proper parties;
  2. Accomplished Affidavit for Delayed Registration at the back of Certificate of Live Birth by the father, mother, or guardian, declaring therein, among other things, the following:
  • name of child;
  • date and place of birth;
  • name of the attending physician;
  • citizenship
  • name of the father if the child is illegitimate and has been acknowledged by him;
  • if legitimate, the date and place of marriage of parents; and
  • reason for not registering the birth within thirty (30) days after the date of birth

3. Payment of ₱ 100.00

FOR MARRIED COUPLE

STEP 1:  Sign the Birth Certificate form Ensure that all information supplied is true and correct.

STEP 2:  Wait for further instruction HIM (Medical Records) personnel will instruct you about the process flow of the certificate.  

STEP 3:  Complete the Clearance Form Settle your accounts at the Cashier. The cashier will sign the Patient’s Clearance Form please present the form to HIM (Medical Records) personnel.

STEP 4:  Receive Records The HIM (Medical Records) Personnel will contact you once the certificate is ready for pick-up. Releasing of the certificate depends on the processing time at the Local Civil Registrar Office

 

IMPORTANT NOTES

The following must be provided to the Health Information Management Office (Medical Records):

If the claimant is the mother of the baby:

  1. A copy of the marriage contract of the parents; (for married couple) &
  2. A copy of one valid I.D. of the mother.

 If the claimant is other than the mother of the baby:

  1. Provide the above requirements;
  2. Authorization letter from the mother of the baby giving permission to release the birth certificate; &
  3. A copy of one valid I.D. of the representative.
